Individual photos are cut out and applied to the certificate, and handwritten entries are in a section entitled "Family Record." Sections include: "Marriages," "Names," "Births," and "Deaths." The images are identified clockwise beginning at 12:00 o'clock: Juan Antonio Córdova and Emelia Elíz Gutierrez Córdova, children Federico, Alejandro, Aurelia, Beatriz, Antonio Elías, Josepha, Elvira, Juan Benito, and Virginia."

From inventory: "Juan Antonio Córdova and his wife Emelia raised their family at Model, Colorado. Juan was born in San Pedro on May 13, 1854 and died on October 19, 1910 at Hoehne, Colorado.
